Actually it hasn't, I wasn't look for why the desingers made him look ugly and deformed I am look for his actual back story why he is ugly and deformed, ie vat of acid?

I think he became obece after becoming a dictator, his deformed head just looks large in the full motion video. God help his mother him having a head like that
