Generally it's only done when the prognosis with other therapies are very poor and it is certain that the source of the problems is exclusively localised in the one hemisphere.

From what I know the main problems resulting from the surgery is paralysis to some extent in the side opposite to the hemisphere removed, as well as visual problems. As for cognitive ability, most young children who have the surgery seem to not lose much if any of their intelligence.

That said, this surgery is very rare, and has only seen an increase in it being performed relatively recently, thus it is still a long time before all the benefits and risks becomes clear. Right now evidence is based more on case studies of individual patients than empirical evidence.
